Point score combining COPD, renal dysfunction, cardiovascular comorbidity, age, diabetes, prior stroke or TIA, and vascular disease.

Demographics and renal function
Age contributes up to 2 points and renal dysfunction contributes up to 2 points.

Renal points are assigned from the estimated glomerular filtration rate.
Clinical components
Each clinical history item contributes 0, 1, or 2 points as defined by the score.
Clinically manifest chronic obstructive pulmonary disease treated with maintenance inhalers.
Documented heart failure diagnosis or LVEF <40%.
Office blood pressure >=140/90 mmHg on at least 2 occasions, or current antihypertensive therapy.
Documented diagnosis, laboratory criteria, or glucose-lowering medication use.
Any prior ischemic stroke or transient ischemic attack.
Prior myocardial infarction and/or peripheral artery disease, including revascularization.
